How to Become a Client
We receive referrals from medical centers and community agencies throughout the San Francisco Bay Area, as well as self-referrals. CMC operates by appointment only. Client appointments are scheduled Friday, Saturday, Sunday, and Monday between 9:30 a.m. and 5:00 p.m. The clinic does not take drop-ins and does not provide home visits. Services are provided at no charge.
Eligibility
You are eligible to become a client if you have received a cancer diagnosis or recurrence within the past 2 years and your financial circumstances preclude you from otherwise being able to afford the cost of complementary therapies that make up integrative care.
You will be asked to complete an in-depth intake form, agree to a release of medical records, and provide documentation of income. Once eligible, you will attend an on-site orientation and receive a clinical assessment with recommendations for type and frequency of integrative care support.
CMC prioritizes appointments for clients who are yet to begin their cancer treatment or are actively undergoing treatment, as well as those who have completed their treatment within the past year. In any case, our integrative care team is prepared to work with you through each phase of your cancer journey.

For Referring Providers
CMC collaborates with medical and social service professionals in hospitals, clinics and community-based organizations throughout the San Francisco Bay Area to ensure that under-served women with cancer are referred as promptly as possible. In addition, cross-referral agreements between us can ensure better care and access to resources while reducing duplication and unnecessary burdens for the women we serve.
Agencies may refer a patient to CMC by completing a CMC Referral Form and faxing it to (510) 645-1886.
